NextGen CHNAs: Advancing Catalytic Community Assessments to Drive Thriving Communities
10:00 a.m. – 12:00 p.m. CT
During this interactive pre-conference workshop, Loma Linda University Health, Fox Valley Data Exchange and the teams supporting The Rippel Foundation’s ReThink Health initiative will guide participants in exploring what it takes to create a future of health and well-being in their communities. Drawing on the Vital Conditions for Health and Well-being Framework and hands-on experiences, this session will share concrete next steps for how hospitals, health systems and their partners can leverage community health assessments to drive systems change, foster collaboration and strengthen community trust.
Loma Linda University Health (LLUH) has a legacy of working deeply in communities to address access to care and social needs. LLUH used this approach, guided by the Vital Conditions framework, to transform the development and implementation of its 2025 Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP). LLUH has also been a key partner in the Inland Empire Vital Conditions Network, a regional “network-of-networks” uniting partners around a shared purpose: building a future where all people and places thrive. This effort exemplifies the power of coordinated, multi-solver strategies that amplify existing strengths and uncover new pathways for collective action.
The Fox Valley Data Exchange is redefining how our region understands and uses data, making it easier than ever to create smart, targeted action across three counties in Wisconsin. The platform organizes its data around the Vital Conditions for Wellbeing and is supporting the work of the Tri-County Health Improvement Coalition, as healthcare and public health leaders come together to assess the needs of their respective communities, set priorities, and establish plans to improve the conditions that enable local residents to be happy, healthy, housed, and thriving.Through case examples from across the country, group exercises and discussion, attendees will learn how to apply these tools to strengthen their regional planning and community health strategies. Attendees will leave with practical tools, peer insights and actionable steps to integrate the Vital Conditions into their own CHNAs and CHIPs, positioning their organizations as catalysts for thriving communities.
Community Immersion Experiences | Wednesday, May 13
Community immersion experiences offer conference attendees the opportunity to visit local hospitals and community organizations to experience programs first-hand. Attendees will leave the hotel by bus and should wear comfortable clothing and shoes.
